국내 노인의 낙상예방 중재의 특성과 효과: 체계적 문헌고찰

Abstract

Purpose

This systematic review aimed (a) to review the characteristics (types, elements, materials, who provided, where, modes of delivery, and intensity) of the fall prevention interventions adopted in studies of the Korean older adults (b) to review their effects by intervention characteristics and measures.

Methods

The data were searched from RISS, Pubmed, CHINAL, EMBASE with key words of ‘aged’, ‘Korean’, ‘accidental falls’. The Joanna Briggs Institute was used to assess methodological quality. Data were extracted for sample nature, the type, element, material, delivery, and intensity of the intervention, measures, and findings.

Results

A total of fifty nine studies were selected for this review. Most studies were conducted with only female older adults (66.1%) and small sample (less than 30 subjects) ( 93.2%). Exercise was the most frequently evaluated intervention type, with a small number of studies including education. Face-to-face (group) was the most frequently evaluated intervention delivery mode; only a small number of studies evaluated non face-to-face intervention. 12 weeks, 3 times a week, and 60 minutes per session were the most frequently evaluated intervention intensity. An exercise intervention was effective in balance and muscle strength. Fall prevention interventions face-to-face (group), delivered at least for 8 weeks, 3 times a week, and 40 minutes per day may be more effective.

Conclusion

This review showed the current status of fall prevention intervention for the Korean older adults. As the literature indicated the exercise can be useful intervention but there need additional evidence from rigorously designed study to determine the effective form of intervention.

서 론

1. 연구의 필요성

낙상은 넘어지거나 떨어져서 몸을 다치는 것으로 부상으로 인한 사망의 주요 원인이다[1]. 특히 노인에서 발생률이 높아 국내 노인의 15.9%가 1년에 한 번 이상 낙상을 경험하며, 이중 64.9% 가 병원에 내원한다[2]. 또한, 노인은 낙상으로 인한 사망이나 중상 위험이 높다[1,3]. 실례로, 국내 일개 종합병원 대상 조사연구에서 낙상 후 응급실을 내원한 노인 중 의학적 처치가 필요한 중등도 이상 손상이 80.9%로 보고되었다[4]. 이러한 노인 낙상의 빈번성과 위험성은 낙상예방 중재의 중요성을 지적하고 있다. 이에 따라 낙상예방을 위해 교육, 운동, 환경 조정에 대해 다양한 중재가 개발되었고, 그 효과에 대해 많은 연구가 수행되었다[5-8].
축적된 연구결과에 대한 고찰을 통해, 낙상예방 중재 연구의 동향을 포괄적이고, 체계적으로 제시하는 것은 새로운 중재 연구를 계획하는 연구자뿐만 아니라, 대상자에게 적합한 중재를 선택해야 하는 간호 실무자 모두에게 유용하다. 낙상 예방 중재 연구에 대해 체계적으로 평가하기 위해 다수의 국내외 문헌고찰 및 메타분석 연구가 수행되었다. 한국인 노인을 대상으로 한 선행연구들은 총 6편으로 4편의 중재 효과에 대한 메타분석 연구(운동이 근력[9], 균형[10], 심리적 변인[11], 체력[11]에 미치는 효과 분석, 다면적 낙상예방 프로그램의 효과 분석[12])와 2편의 문헌고찰 연구(가상현실 운동 중재 고찰[13], 낙상예방 중재의 특성, 결과 변수와 그 효과에 대한 빈도 분포[14])가 이루어졌다. 이들 연구는 대부분, 특정 중재(예, 운동)의 특정 결과 변수(예, 근력)에 대한 효과 분석에 주안점을 두었고, 15편 미만 소수의 연구만을 분석하여 단편적 분석이라는 제한점이 있었다.
즉, 이들 선행 문헌고찰 연구에서는, 어떠한 중재의 유형(예, 교육, 운동), 구성 요소(예, 균형 운동, 근력운동), 사용 도구(예, 탄력밴드, 공), 제공 방법(예, 중재를 제공한 사람, 장소, 형식[대면, 비대면, 집단, 개인]), 강도(기간, 빈도, 시간), 결과 변수, 측정도구가 연구되었는지에 대한 포괄적인 고찰이 되지 않았다. 이에 따라, 어떤 지표의 향상을 위해, 어떤 중재를 어떻게 구성해서 제공해야 하는지를 비교 평가할 근거가 미비한 실정이다. 또한, 체계적 문헌고찰은 대상 인구 집단에 대한 국내외에서 발간된 논문을 포괄적으로 검토하는 것이 주된 목적이나 선행 국내 문헌고찰 연구에서는 국내에서 출판된 연구만 포함되었고[9-14], 해외에서 출판된 한국인 노인을 대상으로한 낙상예방 중재 연구는 포함되지 않았다. 해외에서 수행된 문헌고찰 연구의 경우는 주로 운동 중재의 효과를 분석하였고[15,16], 한국인에 대한 연구결과가 포함되지 않았다. 국가마다 노인의 생활환경, 문화, 복지 체계가 다르므로 한국의 실정에 적합한 중재를 선정하기 위해 한국 노인을 대상으로 발간된 국내외 연구를 포괄적으로 분석할 필요가 있다. 또한, 한국인 노인을 대상으로 한 연구가 해외 학술지에 게재될 수 있고, 이들 연구를 포함하여 문헌고찰을 할 필요가 있다.
따라서 본 연구에서 한국인 노인의 낙상예방을 위한 중재 연구에 대해 다양한 측면(중재의 유형, 구성 요소, 사용 도구, 제공 방법, 강도, 결과 변수, 측정도구)을 포괄적으로 평가하여, 연구자 측면에서는 불필요한 반복 연구들을 피하고, 더욱 정제되고 확대된 후속 연구의 방향성을 제시하고, 간호 실무자 측면에서는 효과적인 중재를 구성하는 데 도움을 주고자 한다.

2. 연구목적

본 연구의 목적은 한국인 노인을 대상으로 한 낙상예방 중재 연구를 고찰하여 (1) 중재의 특성(중재의 유형, 구성 요소, 사용 도구, 제공 방법, 강도), 결과 변수 및 측정도구를 파악하고, (2) 중재의 특성 및 측정도구별로 중재 효과를 비교하여 효과적인 낙상예방 중재의 근거를 제시하고자 한다.

연구방법

1. 연구설계

본 연구는 한국인 노인 대상 낙상예방 중재 연구를 분석하여, 효과적인 중재의 특성(중재의 유형, 구성 요소, 사용 도구, 제공 방법, 강도)을 파악하고, 중재의 목표(결과 변수 및 그 측정도구) 선정의 근거를 확인하기 위해 시행된 체계적 문헌고찰 연구이다. 본 연구의 절차와 결과는 PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analysis) 가이드라인[17]을 활용하여 기술하였다.

2. 분석 대상 논문의 선정기준

선정기준은 PICOSD (Participants, Intervention, Comparison, Outcomes, Study Design)에 따라, (1) 65세 이상 한국인 대상 연구(P), (2) 낙상예방 중재를 포함할 것(I), (3) 대조군(어떠한 실험처치도 하지 않은 집단)이 있을 것(C), (4) 종류와 관계없이 결과 변수가 있을 것(O), (5) 대조군이 있는 실험연구 디자인일 것(SD), (6) 2000년 이후 발간된 논문일 것이었다. 배제 기준은 (1) 치매 또는 특정 질환 환자 대상, (2) 장비를 이용한 환경 조정 중재, (3) 학술대회에 발표된 초록, 학위논문이었다. 특정 질환이 있는 환자 대상 연구는, 대상자의 신체적, 정신적 상태에 적절한 특수한 중재가 이루어져야 해서 제외하였다.

3. 문헌 검색 및 선정 과정

자료 검색은 2020년 9월에, 국내외 데이터베이스(RISS, Pubmed, CHINAL, EMBASE)를 통해 시행하였다. PICO-SD에 따라 주요 용어를 체계적으로 선정하였다. 한국어 주요 용어는 “노인”, “낙상예방”이었고, 영문 논문의 용어는 (1) “Aged” OR “Elderly” OR “Elder” OR “Old”, (2) “Korean” OR “Koreans”, (3) “Accidental Falls” OR “Falls” OR “Falling” OR “Falls, Accidental” OR “Accidental Fall” OR “Fall, Accidental” OR “Slip and Fall” OR “Fall and Slip”이었다.
검색된 문헌들은 중복을 제거하여 1차 분류하고, 선정 및 제외 기준에 따라 제목과 초록을 검토하여 2차 분류하였다. 마지막으로 전문을 검토하여 최종 분석 대상을 선정하였다. 자료 검색 과정은 연구자 2인이 시행하여 오류를 방지하였다. Figure 1에 제시한 바와 같이, 검색된 문헌은 총 1,616편이었다. 중복 349편을 제외하고 제목과 초록을 검토하여 127편의 논문이 선택되었고, 전문 검토를 통하여 최종 논문 59편을 선정하였다[18-76].

4. 문헌의 질 평가

The Joanna Briggs Institute (JBI) 도구[77]로 논문의 질을 평가하였다. 유사 실험연구 평가도구는 9개 항목(총점 0~9점)으로 구성되며, 각 문항은 원인과 결과를 분명히 제시하였는가, 중재 전 그룹 간 동질성을 확인하였는가, 외생변수의 개입 및 그에 대한 통제를 명확히 설명하였는가, 대조군이 있는가, 사전 ․ 사후를 측정하였는가, 탈락자 처리에 대해 기술하였는가, 실험처치 전, 후에 동일한 효과 측정 방법을 적용하였는가, 신뢰도와 타당도가 검증된 측정도구를 사용하였는가, 적절한 통계 방법을 사용하였는가를 평가한다. 무작위 실험연구 평가 도구는 13개 항목(총점 0~13점)으로 구성되며, 유사 실험연구 문항에 추가로 대상자를 무작위 할당하였는가, 무작위 할당, 대상자, 실험자, 그리고 측정자를 맹검하였는가, 무작위로 추출된 그룹에서 분석하였는가, 실험 설계가 적절하였는가를 평가한다.
통합적 고찰에서 선정된 연구의 질이 낮은 경우에 문헌을 제외하기보다는 의미 부여를 낮게 하는 것이 보편적이라고 한 선행연구[78]를 근거로, 포괄적으로 연구의 동향을 분석하기 위해 본 연구에서는 모든 연구를 분석 대상에 포함하였다.

5. 자료 추출 및 분석

자료 추출 및 분석을 위하여 Excel 프로그램(2019. Microsoft)을 사용하였다. 1인의 연구자가 저자, 표본 특성, 중재 특성(중재의 유형, 구성 요소, 사용 도구, 제공 방법, 강도), 결과 변수 및 측정도구, 중재 결과에 대한 정보를 추출하였고, 또 다른 연구자가 재확인하여 오류가 없도록 하였다. 중재의 유형은 운동, 교육으로 분류하였고, 구성 요소는 근력(muscle strength), 균형(balance), 유연성(flexibility), 유산소(aerobic) 등으로 세부적으로 분류하였다. 사용 도구는 탄력밴드, 공 등으로 분류하여 분석하였다. 제공 방법에서 중재를 제공한 사람은 연구자, 훈련된 연구보조원, 고용된 전문가로 분류하였고, 장소는 주거 복지시설(양로원), 의료 복지시설(요양원, 노인 의료 시설, 보건소), 여가 복지시설(경로당, 복지시설, 체육관, 노인학교, 건강증진센터, 체력 단련실), 재가 시설(방문간호)로 분류하여 분석하였고, 형식은 대면, 비대면, 집단, 개인으로 분류하였다. 중재 강도는 기간, 빈도, 시간으로 분류하였다. 결과 변수는 관찰자가 객관적으로 측정한 변수와 대상자가 주관적으로 측정한 변수로 나누고, 세부적인 변수 영역 분류 작업을 거쳐, 변수별로 측정도구를 분류하였다. 중재의 효과는 중재 특성별(중재의 유형, 구성 요소, 사용 도구, 제공 방법, 강도), 측정도구별로 나누어 분석하였다. 분석은 패턴을 분석하는 질적 분석에 주안점을 주었다.

연구결과

1. 문헌 선정 결과 및 연구의 특성

선정된 연구는 2000년부터 2019년까지 발간된 총 59편(유사 실험연구 28편, 무작위 실험연구 31편)이었다. 본 연구에서 디자인 별로 차이를 알기 위해, 각 디자인 별로 분석을 시행하였다(Tables 1, 2). 유사 실험연구는 여성만을 대상(n= 15)으로 한 연구가 가장 많았고, 대다수 연구가 30명 미만(n= 27)으로 표본 수가 적었다. 무작위 실험연구도 대다수 연구가 여성만을 대상(n=24)으로 하였고, 30명 미만(n=28)으로 표본 수가 적었다. 두 디자인 모두 대조군은 실험군과 유사한 표본 수로 구성되었고, 연령도 유사하게 모집되었다.

2. 논문의 질 평가 결과

본 연구에서 무작위 할당 여부에 따라 유사 실험연구, 무작위 실험연구로 나누어 평가하였다. 유사 실험연구의 질 평가 점수는 5에서 9점으로 분포하여 모든 연구가 중간 이상의 점수를 받았다(Table 1). 대부분의 연구(n=26)에서 측정도구의 신뢰도와 타당도에 대한 사전 검증이 되지 않았고, 과반수(n=18)에서 사전 동질성을 확인하지 않았다. 최하점은 받은 연구 1편(5점)은 디자인에서 원인과 결과 간의 관계 기술이 미비하거나, 외생변수의 개입 및 그에 대한 통제를 명확히 설명하지 않은 논문 작성상의 문제가 추가로 있었다. 결론적으로, 연구의 질이 유사하고, 문제가 되는 영역도 유사하여 심각한 비뚤림이 있는 연구는 없다고 판단하였다.
무작위 실험연구의 질 평가 점수는 4에서 10점으로 분포하여, 대부분(n=24)이 중간 미만의 점수를 받았다(Table 2). 점수가 낮은 이유는 대부분의 연구가 무작위 할당(n=27), 대상자(n=31), 실험자(n=30), 그리고 측정자(n=25)를 맹검하지 않았고, 도구의 신뢰도와 타당도를 보고하지 않았고(n=29), 과반수(n=18)에서 사전 동질성 검정이 미비하였기 때문이었다. 최하 점수를 받은 1편(4점)은 외생변수의 개입 및 그에 대한 통제를 명확히 설명하지 않은 논문 작성상의 문제가 추가로 있었다. 무작위 실험연구는 연구의 질이 모두 낮고, 또한 문제 영역도 유사하다는 점에서, 모든 연구가 유사한 수준의 비뚤림이 있다고 판단하였다. 이러한 결과는 개개 연구의 타당성을 위협할 수 있으나, 비뚤림이 높은 연구들이 특정한 패턴의 결과를 보이지 않아, 전체적으로 연구 동향을 파악하는 본 연구결과에 영향을 크게 미치지 않았다고 판단되며, 또한 본 연구에서는 선행연구 디자인상의 제한점을 평가하여 추후 연구의 방향을 제안하고자 하였다.

3. 낙상예방 중재의 특성(중재의 유형, 구성 요소, 사용 도구, 제공 방법, 강도)

유사 실험연구(n=28)의 중재 유형은 1개의 연구를 제외한 모든 연구가 운동 중재를 포함하였고, 7편의 연구에서 교육 중재를 추가로 활용하였고, 1편의 연구에서 교육 중재만을 수행하였다. 구성 요소는 근력운동(n=15)과 균형 운동(n=7)이 가장 많았으며, 유산소 운동을 포함한 중재가 5편(걷기 1편)의 연구에서 시행되었고, 유연성 운동을 포함한 중재가 3편이 있었다. 타이치(n=2), 필라테스(n=2) 등과 같은 다양한 운동 요소를 포함한 정형화된 프로그램을 활용하는 연구도 있었다. 2개 이상의 구성 요소를 같이 제공한 연구는 근력 ․ 균형 운동(n=4)이 가장 많았다. 사용 도구는 탄력밴드(n=7)가 가장 많았고, 제공 방법으로 중재를 제공한 사람은 고용된 전문가(n=4)가 가장 많으며, 장소는 여가 복지시설에서 많이 이루어졌다(n=17). 형식은 대다수의 연구가 대면으로 집단을 대상(n=25)으로 중재를 수행하였다. 강도는 기간이 6~16주, 빈도가 주 1~10회, 시간이 20~90분으로 다양하였고, 12주(n=12) 간, 주 3회(n=15), 60분(n=14) 씩이 가장 빈번하였다.
무작위 실험연구(n=31)의 중재 유형은 모든 연구가 운동 중재를 포함하였고(n=31), 4편의 연구에서 교육 중재를 추가로 활용하였다. 구성 요소는 근력운동(n=19)과 균형 운동(n=14)이 가장 많았으며, 유연성 운동이 2편, 유산소 운동(걷기)이 1편의 연구에서 시행되었다. 몇몇 연구에서 댄스스포츠(n=3), 타이치(n=3), 한국무용(n=2), 수중운동(n=2) 등과 같은 정형화된 프로그램을 활용하였다. 2개 이상의 운동 구성 요소를 같이 제공한 연구는 근력 ․ 균형 운동(n=7)이 가장 많았다. 사용 도구는 탄력밴드(n=10), 공(n=4)이 가장 많았다. 제공 방법으로 중재를 제공한 사람은 고용된 전문가(n=4)가 가장 많았으며, 장소는 대다수 연구가 여가 복지시설에서 이루어졌다(n=25). 형식은 대다수의 연구가 대면으로 집단을 대상(n=25)으로 수행되었다. 1편의 연구에서 비대면 중재만을 개인을 대상으로 시행하였다. 강도는 기간이 4~24주, 빈도가 주 2~7회, 시간이 20~100분으로 다양하였고, 12주(n=11) 간, 주 3회(n=22), 60분(n=14) 씩이 가장 빈번하였다.
두 실험 디자인 모두 중재 유형은 교육 중재가 일부 연구(유사 실험 8편, 무작위 실험 4편)에서 활용되었으며, 교육의 내용은 일반적인 내용(낙상의 정의, 원인, 예방)과 환경 조정에 대한 내용(환경의 중요성, 주택 개조 제공 유형, 주택 개조를 위한 지원)으로 구성되었다. 구성 요소는 다양하였으나 대부분의 연구(유사 실험 19편, 무작위 실험 22편)가 하체 운동에 집중하였다. 사용 도구는 일부 연구(유사 실험 1편, 무작위 실험 2편)에서 운동 장비를 이용하였다. 제공 방법으로 중재를 제공한 사람은 연구자(유사 실험 3편, 무작위 실험 1편), 훈련된 연구보조원(유사 실험 2편, 무작위 실험 1편)가 일부 연구에서 보고되었다. 장소는 주거 및 의료 복지 시설, 재가 시설에서 이루어진 연구는 거의 없었다. 특정 질환을 대상으로 한 연구를 제외하여, 모든 연구가 지역사회에서 이루어졌다. 형식은 대부분의 연구(유사 실험 28편, 무작위 실험 30편)가 대면이었고, 비대면인 경우 집단을 대상으로 한 연구는 없었다. 강도는 24주가 가장 긴 기간이었으며, 52주(1년) 즉, 장기간의 연구는 없었다.

4. 결과 변수의 종류 및 측정도구

다양한 결과 변수가 중재 효과를 파악하기 위한 지표로 사용되었으며, 본 연구에서 객관적 변수를 총 10영역으로 분류하였고(Table 3), 주관적 변수는 총 14영역으로(Table 4) 분류하여 조사하였다. 객관적 변수로 균형(n=44), 근력(n=44), 보행(n=30), 민첩성/협응력(n=25), 유연성(n=24), 신체 조성(n=14) 순으로 많이 연구되었다. 소수의 연구에서 심폐기능, 혈액학적 지표, 고유수용성 감각, 시지각 등이 연구되었다. 균형 변수의 측정도구로는 외발서기(n=25), 근력 측정에는 의자에 앉았다 일어서기(n=28), 보행 측정에는 걷기, 발 스테핑(n=19)이 가장 많이 활용되었다. 민첩성/협응력 측정에는 의자에서 일어나 걸어갔다 돌아오기, 양발을 벌렸다 오므리는 동작, 8자 보행을 포함한 Senior Fitness Test 도구[79](n=23)가 가장 많이 활용되었고, 유연성 측정에는 앉아서 윗몸 앞으로 굽히기(n=20), 신체 조성 측정에는 체중(n=11)이 가장 많이 활용되었다.
주관적 변수로는 낙상 효능감(n=15), 낙상 두려움(n=8), 균형 자신감(n=5), 건강 상태(n=3), 낙상 빈도(n=3), 낙상예방(n=3) 순으로 많이 연구되었다. 일상생활능력, 우울, 자아탄력성, 낙상 위험, 허약 수준, 통증, 주관적 행복감과 같은 간접적인 지표도 소수의 연구(각 3편 미만)에서 변수로 선정되었다. 낙상 효능감 측정도구로는 Tinetti 척도[80](n=10), 낙상 두려움 측정도구로는 Tideiksaar가 개발한 척도[81](n=6)가 가장 많이 활용되었고, 균형 자신감 측정도구는 Activities-specific Balance Confidence [82](n=4)가 가장 많이 활용되었다. 이외에도 다양한 측정도구들이 연구에서 활용되었고, 5개의 도구가 저자가 개발한 도구였다.

5. 중재 특성별, 측정도구별 중재 효과의 차이

두 디자인 모두에서 중재의 유형을 보면 운동을 포함한 중재는, 거의 모든 연구에서 근력 증가(유사 실험 22편 중 20편, 무작위 실험 22편 중 20편)와 균형감각 향상(유사 실험 18편 중 14편, 무작위 실험 26편 중 23편)에 효과가 있었다. 교육을 함께 적용한 모든 운동 중재 연구에서 근력(유사 실험 6편, 무작위 실험 3편)과 균형(유사 실험 5편, 무작위 실험 4편)에 효과가 있었다. 교육만을 중재로 한 연구는 단 1편[36]으로 이 중재의 효과를 확인하기에는 근거가 부족하였다.
구성 요소를 보면 균형 운동을 포함한 중재는 균형감각 향상에 근력운동보다 일관성 있게 효과가 있었고, 근력운동을 포함한 중재는 근력 증가에 균형 운동보다 일관성 있게 효과가 있었다.
사용 도구로 탄력밴드를 가장 빈번히 이용하였고, 근력 증가(유사 실험 7편 중 6편, 무작위 실험 8편 중 7편)와 균형감각 향상(유사 실험 5편 중 3편, 무작위 실험 9편 중 6편)에 효과가 있었다.
제공 방법을 보면 중재를 제공한 사람은 전문가가 제공한 거의 모든 연구에서 근력(유사 실험 3편 중 2편, 무작위 실험 3편 중 3편), 균형(유사 실험 2편 중 2편, 무작위 실험 4편 중 4편)에 효과가 있었다. 연구자가 제공한 모든 연구에서도 근력(유사 실험 3편, 무작위 실험 1편), 균형(유사 실험 2편, 무작위 실험 1편)에 효과가 있었다. 장소는 여가 복지시설에서 많이 이루어졌고, 근력 증가(유사 실험 13편 중 13편, 무작위 실험 17편 중 15편), 균형감각 향상(유사 실험 9편 중 8편, 무작위 실험 21편 중 18편)에 효과가 있었다. 형식은 대부분의 연구가 집단을 대상으로 대면으로 중재를 제공하였고, 이러한 제공 유형만을 사용한 연구는 대다수의 연구에서 근력 증가(유사 실험 16편 중 14편, 무작위 실험 19편 중 17편)와 균형감각 향상(유사 실험 15편 중 11편, 무작위 실험 20편 중 17편)에 효과가 있었다. 소수의 연구에서 개인을 대상으로 대면 중재를 제공하였고, 이러한 제공 형식만을 사용한 모든 연구에서 근력(유사 실험 3편, 무작위 실험 1편)과 균형(무작위 실험 4편)에 효과가 있었고, 개인과 집단을 병합한 대면 중재만을 제공한 모든 연구에서도 근력 ․ 균형(유사 실험 3편, 무작위 실험 1편)에 효과가 있었다. 비대면 중재만을 사용한 연구는 단 1편[76]이기 때문에 효과는 확인하기 어려웠다.
강도를 보면 중재 기간으로 가장 많이 연구된 기간인 12주 중재는, 대다수의 연구에서 근력 증가(유사 실험 9편 중 7편, 무작위 실험 7편 중 7편)와 균형감각 향상(유사 실험 10편 중 6편, 무작위 실험 11편 중 9편)에 효과가 있었다. 중재 기간이 8주인 경우에도 대다수의 연구에서 근력 증가(유사 실험 7편 중 7편, 무작위 실험 8편 중 7편)와 균형감각 향상(유사 실험 5편 중 5편, 무작위 실험 6편 중 5편)에 효과가 있었다. 중재 기간이 6주인 경우에도 근력과 균형에 효과가 있었으나 단 1편[61]으로 6주의 중재 기간의 효과를 확인하기에는 근거가 부족하였다. 중재 빈도는 주 3회를 가장 많이 연구하였고, 대다수의 연구에서 근력 증가(유사 실험 12편 중 11편, 무작위 실험 15편 중 14편)와 균형감각 향상(유사 실험 14편 중 10편, 무작위 실험 19편 중 17편)에 효과가 있었다. 중재 빈도가 주 1회인 경우에도 근력과 균형에 효과가 있었으나 단 1편[41]으로 주 1회의 중재 빈도의 효과를 확인하기에는 근거가 부족하였다. 중재 시간은 60분이 가장 많이 연구되었고, 대다수의 연구에서 근력 증가(유사 실험 10편 중 8편, 무작위 실험 10편 중 8편)와 균형감각 향상(유사 실험 10편 중 8편, 무작위 실험 11편 중 9편)에 효과가 있었다. 중재 시간이 40분인 경우에도 근력, 균형에 효과가 있었다(유사 실험 1편, 무작위 실험 1편).
표 34에 도구별로 중재 효과 검정 결과를 제시하였다. 많이 사용된 도구(5편 이상 연구) 중에서 70% 이상 연구에서 일관성 있게 효과가 있다고 보고한 객관적 도구는 균형 영역에서 Berg 균형 척도[83], 기능적 팔뻗기, 외발서기, 근력 영역에서 하지(발목, 고관절, 무릎, 다리) 근력 측정, 의자에 앉았다 일어서기, 민첩성/협응력 영역에서 의자에서 일어나 걸어갔다 돌아오기, 양발을 벌렸다 오므리는 동작, 8자 보행, 보행 영역에서 보폭, 걷기, 발 스테핑이었다. 신체 조성, 유연성 영역에는 일관성 있게 효과가 있는 도구가 없었다. 주관적 도구로는 낙상 효능감 영역에서 Tinetti 낙상 효능감 척도[80]가 9편의 연구에서 일관성 있게 중재 효과가 있었고, 낙상 두려움 영역에서 일관성 있게 효과가 있는 도구는 없었다. 다른 영역에서는 각 도구의 활용 빈도가 5회 미만으로 낮았다.

논 의

본 연구는 한국인 노인을 대상으로 한 낙상예방 중재 연구 59편을 분석하여, 지금까지 연구된 중재의 특성(중재의 유형, 구성 요소, 사용 도구, 제공 방법, 강도), 결과 변수와 측정도구를 체계적으로 분류하여 현황을 확인하였고, 중재의 특성 및 측정도구별로 효과의 차이를 보이는 연구결과 상의 패턴이 있는지를 검토하였다. 중재 유형은 운동 중재가 가장 많이 연구 되었고, 구성 요소는 근력, 균형이 가장 많았으며, 사용 도구는 탄력밴드가 가장 많았다. 제공 방법으로 중재를 제공한 사람은 고용된 전문가가 가장 많았고, 장소는 여가 복지시설이 가장 많았고, 형식은 집단을 대상으로 대면으로 시행한 중재가 가장 많았다. 강도(기간, 빈도, 시간)는 12주간, 주 3회, 회당 60분간이 가장 많았다. 다양한 결과 변수와 객관적, 주관적 도구가 연구되었다. 결과 변수로 가장 많이 연구된 것은 균형과 근력이었다. 균형 측정에는 외발서기, 근력 측정에는 의자에 앉았다 일어서기가 가장 많이 사용되었다. 이상과 같은 중재의 특성과 결과 변수를 분석한 결과는 이전에 시행된 한국인 노인 대상 체계적 문헌고찰 연구결과와 중재 유형, 구성 요소, 결과 변수에 대한 빈도 분포[13,14]에서 유사하였다. 강도 중 시간에 대한 결과는 일치하였으나, 기간, 빈도는 8주간, 주 1회가 많았다는 선행연구결과[14]와 다소 차이가 있었다. 이러한 결과의 차이는 표본의 차이로 인한 것이며, 본 연구의 결과가 광범위한 문헌고찰을 통해 보다 확정적이고 신뢰할 수 있다.
중재의 특성에 따른 효과의 패턴을 분석한 결과, 중재의 유형 중 운동 중재는 균형과 근력 향상에 도움을 주었으며, 구성 요소 중 균형 운동은 균형 향상에, 근력운동은 근력에 더 도움을 주었다. 하지의 균형과 근력이 모두 낙상 예방에 필요하므로 두 요소를 모두 포함한 운동 방법을 구성할 필요가 있다. 운동 중재 효과를 메타 분석한 한국인 노인 대상 연구[10]에서 하지 근력운동은 균형에 효과가 있고, 11개 국가(네덜란드, 미국, 뉴질랜드 등)의 노인 대상으로 한 연구[84]에서도 하지 근력운동은 근력과 균형에 효과가 있다고 하여 일관성 있는 중재의 유형의 효과를 확인하였다. 특정한 조건(환경이나 개인의 상태)에 있는 노인에게 적합한 중재의 유형과 구성 요소를 결정하는 추가적인 연구가 필요하다.
중재의 유형은 교육 중재를 같이하는 것이 도움이 되는 것으로 추정이 된다. 15개 국가(브라질, 호주, 미국 등)의 노인 대상으로 한 연구[85]에서도 운동과 교육을 함께한 중재는 낙상자 감소에 효과가 있다고 하였다. 그러나 본 연구결과 및 선행연구에서도 교육 중재의 단일 효과는 확실하지 않아 추가적인 연구가 더 필요하다.
제공 방법 중 형식에서 집단을 대상으로 대면 중재를 제공하는 방법이 가장 많이 연구되었고, 일관성 있게 효과적인 것으로 확인되어 지역사회에서 경제적인 집단 중재를 활용할 수 있는 근거를 제시하였다. 그러나 대면 중재 중 개별 중재가 집단 중재보다 더 효과적인지에 대한 추가 연구를 통해, 개별 중재가 필요한 인구 집단을 선별하는 근거를 마련할 필요가 있다. 또한, 비대면 중재만을 연구한 한국인 노인 대상 연구는 단 1편이어서 추가 연구가 필요하다. 특히 외출과 타인 접촉을 자제해야 하는 장기적인 감염질환의 유행과 같은 사회적 문제가 있는 시대를 대비하여, 노인을 대상으로 비대면 중재의 효과를 확인할 필요가 있다. 예를 들어, 온라인 홈트레이닝 애플리케이션[86], 원격 운동과 교육 중재[87]가 동기 부여와 낙상예방에 도움이 될 수 있을지 연구해 볼 필요가 있다. 호주의 병원에서 퇴원한 노인을 대상으로 시행한 DVD (Digital Video Disc)를 이용한 가정에서의 하지 근력 ․ 균형 운동은 낙상 감소에 효과가 확실하지 않아[88], 상황에 적절한 비대면 중재를 개발할 필요가 있다.
강도별 효과의 차이 분석에서, 최소 중재 기간은 8주, 중재 빈도는 최소 주 3회, 중재 시간은 각 회당 40분인 경우 일관성 있게 균형과 근력 향상에 도움이 되었다. 이 결과는 한국인 노인 대상 연구에서 운동 기간과 시간은 근력에 영향을 주지 않았고[9], 심리적 변인과 체력에 영향을 주지 않았다[11]는 결과와 차이가 있었다. 이는 선행연구에서는 중재 기간이나 빈도 또는 시간이 증가함에 따라 효과 크기의 차이를 분석한 결과이므로 중재 기간, 빈도, 시간별 효과 크기를 각각 분석하는 추가적인 연구가 더 필요하다.
결과 변수 및 측정도구에 대한 분석 결과는 운동 중재에 대한 효과 측정지표와 방법 선정에 대한 근거를 제공하였다. 즉 운동의 효과의 지표는 근력과 균형이며, 각 변수를 측정하는 민감한 도구는 근력 영역에서는 하지(발목, 고관절, 무릎, 다리) 근력 측정, 의자에 앉았다 일어서기, 균형 영역에서는 버그 균형 척도, 기능적 팔뻗기, 외발서기였다. 측정도구에 대한 분석에서 일관성 있게 유의하다고 보고된 도구가 민감도가 높다고 판단할 수 있다. 6개 국가(오스트리아, 대만, 영국 등)의 노인을 대상으로 한 연구[89]에서는 외발서기, 기능적 팔 뻗기, 의자에 앉았다 일어서기, 의자에서 일어나 걸어갔다 돌아오기에 효과가 있었고, 중재 효과가 있는 변수는 균형, 근력, 민첩성/협응력으로 확인되었다.
본 연구에서 연구의 방법론적인 문제점도 발견하였다. 대다수의 연구가 운동이 효과가 있다고 보고하였으나, 보다 잘 디자인된 연구에서 확정적인 근거가 필요하다. 대다수의 연구에서 표본 크기가 30명 미만으로 매우 작아 외적 타당도가 낮고, 맹검이 이루어지지 않아 대상자, 중재자, 결과 평가자의 기대감, 의도가 연구결과에 영향을 미칠 수 있어 내적 타당성에 문제가 있을 수 있다. 또한, 대부분의 연구에서 도구의 타당도와 신뢰도에 대한 정보가 부족하여 내적 타당성을 위협하고 있다. 질 평가 결과에서 나타난 바와 같이, 특히 무작위 실험연구에서 질이 낮은 연구가 많았다. 표본의 구성에서 남성만을 대상으로 하거나 따로 분석한 연구가 없어 남성의 특성을 반영한 중재 연구가 필요하다. 제공 방법 중 장소는 여가 복지시설에서 이루어진 연구가 가장 많아, 취약 노인(복지 시설 이용을 꺼리거나 경제적으로 어려운 노인)을 대상으로 한 중재 연구도 필요함을 알 수 있었다.
낙상예방 중재의 최종적인 목적은 낙상, 낙상자 수를 감소시키는 데 있다. 그러나 3편만이 결과 변수로 낙상 빈도를 측정하였다. 해외 노인을 대상으로 한 연구와[15,16,90,91] 비교하면, 한국인 노인 대상 연구에서 낙상예방 중재의 직접 효과를 측정하는 연구 및 낙상예방 중재의 지속성을 측정하는 연구는 매우 미흡하였다. 추후, 낙상 또는 낙상자 빈도 등을 결과 변수로 포함하고, 장기간에 걸친 연구를 통해 낙상예방 중재효과의 지속성에 대한 연구가 필요하다.
본 연구의 결과를 해석하고 적용하기 위해서는 다음과 같은 제한점을 고려해야 할 것이다. 본 연구는 효과 크기를 통계적으로 분석하지 않아 효과 크기를 메타분석한 연구가 추가로 필요할 것이다. 본 연구는 다양한 낙상예방 중재, 측정 변수 ․ 도구를 제시하였지만, 각각의 디자인별 효과 비교는 논문의 수가 적어 연구결과 적용에 신중해야 하며, 추가 연구가 필요하다. 또한, 본 연구는 대상자에서 치매 또는 특정 질환 환자를 제외하였기에 이들 노인을 대상으로 한 연구들의 고찰도 필요하다. 이와 같은 제한점에도 불구하고 본 연구는 한국인 노인의 낙상을 예방하기 위해 적용된 낙상예방 중재의 중재 특성 및 측정도구별로 효과, 측정한 모든 변수와 도구를 최초로 분석한 체계적 문헌고찰 연구라는 점에서 간호학적 의의가 있다.

결론 및 제언

본 연구는 한국인 노인의 낙상예방 중재 연구의 현황을 제시하였고, 중재 유형은 운동이, 중재 제공 방법 중 형식은 집단을 대상으로 대면이, 중재 기간은 최소 8주, 빈도는 최소 주 3회, 중재 시간은 최소 40분 이상 적용한 중재는 근력 증대와 균형감각 향상에 도움이 될 수 있음을 확인하였다. 본 연구결과는 최소 중재의 강도(기간, 빈도, 시간)를 결정하는 중요한 정보를 제공하였으며, 반복 연구를 통해 확정적 근거를 제공할 필요가 있다. 또한, 본 연구는 선행연구결과에 추가로 이용 가능한 다양한 결과 변수와 상대적으로 민감도가 높은 도구에 대한 정보를 제공하여 결과 변수와 측정도구를 선정하는 근거 자료를 제공하였다. 본 연구결과는 지역사회에서 노인의 낙상예방 중재를 선택, 적용하기 위한 기초자료로 활용되기를 기대한다. 그러나 연구설계상의 제한점을 고려하면, 보다 확정적 근거 마련을 위한 대규모의 잘 디자인된 추가 연구가 필요하다.
