This study was done to develop competencies and core curriculum for gerontological nursing education in programs towards a Bachelor of Science in Nursing (BSN).
From June 2015 to January 2016, competencies and core curriculum from gerontological nursing education in foreign countries were analyzed using a literature review, along with an analysis of the current gerontological nursing curriculum in Korean BSN programs. Competencies and core curriculum of undergraduate nursing education for gerontological nursing were determined through expert panel discussions in the Korean Gerontological Nursing Society.
Data were collected from 30 BSN programs in Seoul, Kyunggido, Kangwondo, Chungchungdo, Kyungsangdo, and Jeollado. Compared with the core curriculum for gerontological nursing recommended by the National Gerontological Nursing Association, there were deficiencies in legal/ethical issues, administrative issues, nursing research in gerontological nursing, and care and service delivery. From the data and panel discussion, 13 competencies and core curriculum including 6 contents, 32 programs, and 197 learning objectives were found to be important.
Use of the recommended curriculum from this study can address deficiencies in baccalaureate gerontological nursing education, but it is also necessary to develop textbooks based on the recommended 13 competencies and core curriculum including 6 contents, 32 programs, and 197 learning objectives.
우리나라는 평균수명의 증가에 따라 노인인구가 2000년 7.2%에서 2015년 12.8%로 증가하였으며, 2026년 20.0%, 2058년에는 40.0%를 초과할 전망이다[
국내 간호교육계에서는 1981년 노인간호학 교과목이 처음 개설되었으며, 이후 노인간호학의 중요성이 증가되면서 1997년 42개 4년제 간호대학의 교과과정에 대한 연구결과 전체의 90.5%가 노인간호학을 독립된 과목으로 교육하고 있는 것으로 보고되었다[
이제 노인간호학은 자리매김을 더욱 확고히 할 시점에 있다. 병원, 노인요양시설, 보건소, 노인복지센터, 주간보호센터, 방문간호서비스 등 다양한 노인간호 서비스에 대한 요구가 증가됨에 따라 간호사의 역할 또한 커지고 있다. 이에, 고령화 사회로의 급격한 이행과 함께 노인간호에 대한 수요 증대와 질적 향상이 요구되는 현시점에서 노인간호에 대한 올바른 태도와 지식을 함양하기 위한 간호교육에서의 노력이 더욱 필요하다[

따라서, 사회 요구에 부응하는 노인간호를 수행할 수 있는 간호사의 양성을 위해서는 학부 노인간호 교육을 통해 달성해야 하는 핵심역량과 노인간호의 방향 제시가 될 수 있는 표준교과과정의 개발이 시급하다. 이에 본 연구는 학부 노인간호학 교육의 효율적이고 합리적인 정착을 위해 핵심역량과 표준교과과정을 제시할 것이다.
본 연구는 문헌고찰을 통해 국외 노인간호학 교육의 핵심역량과 교과과정을 분석하고, 국내 간호교육기관에서의 노인간호학 교과과정의 현황을 분석한 후 전문가 패널의 논의를 통해 학부 노인간호학 교육의 핵심역량과 표준 교과과정 마련을 위해 시도되었으며 구체적인 목표는 다음과 같다.

본 연구는 한국 간호대학(간호학과, 간호학부)의 학부 노인간호학 교과과정 표준화를 위한 핵심역량과 표준 교과과정 및 학습목표 도출을 위해 문헌고찰, 횡단적 조사 및 분석, 전문가 패널의 방법을 혼용한 혼합연구설계를 이용하였다.
본 연구는 국내 4년제 노인간호학 교과과정을 분석 대상으로 하였으며 전국의 4년제 간호대학 중 노인간호학이 개설되어 있는 간호교육기관 120개교에 이메일로 강의계획서를 요청하였다. 자료 요청 시 이메일에 회신하는 것을 동의서로 대신하는 것을 명시하였다. 총 30개교가 회신하여 응답률은 25.0%였다. 패널토의에 참여한 전문가는 17개 간호대학과 1개 요양병원 소속인 노인간호학회의 전현직 이사 24명이다.
본 연구를 위한 자료수집기간은 2015년 6월부터 2016년 1월까지이다. 전국의 4년제 간호대학에 이메일을 통하여 먼저 본 연구의 필요성과 목적에 대해 설명하여 연구협조의 동의를 구하였으며, 동의를 한 간호교육기관의 노인간호학 담당 교수자를 통해 자료분석을 위한 강의계획서를 받았다. 강의계획서를 통하여 불확실한 내용 및 부족한 부분은 유선을 통해 정확히 확인하여 자료의 오류를 방지하였다. 수집된 자료는 기술통계를 사용하여 분석하였다.
국외의 문헌분석과 국내 노인간호학 교과과정 현황분석을 토대로 2회의 노인간호학회 전문가(제9기 노인간호학회 이사 14명) 패널토의를 통해 학부 노인간호학의 핵심역량과 교과과정의 대분류와 중분류의 초안을 마련하였다. 이후 핵심역량과 교과과정의 대분류와 중분류의 초안은 노인간호학회 회원 전체를 대상으로 시행된 워크숍을 통해 수정되었다. 노인간호학회 전문가(제 10기 노인간호학회 이사 14명) 패널은 총 3회의 워크숍을 통해 수정된 핵심역량과 교과과정의 대분류와 중분류를 바탕으로 핵심역량과 교과과정의 대분류와 소분류를 보완하고, 학습목표을 도출하였다. 마지막으로 전국의 노인간호학 교수에게 핵심역량, 교과과정의 대분류, 중분류, 학습목표에 대한 의견수렴을 하여 최종 핵심역량, 교과과정의 대분류, 중분류, 학습목표를 도출하였다.
미국의 American Association of Colleges of Nursing (AACN)은 학부 노인간호학에 필요한 19개의 핵심역량을 제시하였고, 각 핵심역량을 달성하기 위해 교과과정에 포함될 내용을 제시하였다[

본 연구결과 학부 노인간호학에서 달성해야 할 핵심역량은 13개가 도출되었고, 교과과정에서 포함되어야 할 표준 교과과정으로 대분류 6개, 중분류 32개, 학습목표 197개가 도출되었다(
대분류로는 고령화 사회와 노인간호 이해, 노인간호 윤리와 법, 노인간호 서비스전달체계, 노인건강증진, 노인의 안전과 환경, 노인의 건강문제와 간호 영역으로 총 6개 영역이 제안되었다. 고령화 사회와 노인간호 이해 대분류에는 노인인구의 특성, 정상적인 노화과정, 노화이론, 노인과 가족, 노인간호의 기능과 역할이 중분류로 포함되었고, 노인간호 윤리와 법 영역에서의 중분류는 노인간호의 윤리적 측면, 노인간호의 법적 측면, 노인학대를 포함한다. 노인간호 서비스전달체계 영역에서의 중분류는 노인간호서비스의 연속성, 노인장기요양보험제도, 급성기 노인간호, 재가노인간호, 요양시설노인간호이며, 노인건강증진 영역에서의 중분류는 건강검진, 건강교육 및 상담, 자기관리를 포함하고, 노인의 안전과 환경 영역에서는 노인안전관리 이해, 낙상예방 및 관리, 억제대 비사용 접근, 안전한 약물사용을 중분류로 포함한다. 노인의 건강문제와 간호영역에서는 포괄적 노인사정, 노인증후군, 치매, 섬망, 우울증 및 자살, 요실금, 변실금, 전립선비대증, 요로감염, 욕창, 영양불량 및 탈수, 수면장애, 통증, 노인성 난청, 백내장, 황반변성, 친밀감과 성, 임종과 사별의 중분류를 다루도록 하였다. 각 중분류에 따라 3~11개의 학습목표를 제시하였다.
본 연구는 학부 노인간호학의 핵심역량과 표준교과과정을 개발하기 위해 노인간호학회에 의해 수행되었다. 문헌고찰을 통해 국외 노인간호학 교육의 핵심역량과 교과과정을 분석하고, 국내 간호교육기관에서의 노인간호학 교과과정의 현황을 분석한 후 노인간호학회 전문가 집단의 논의를 통해 학부 노인간호학 교육의 핵심역량과 표준 교과과정을 도출하였다. 국내 간호학 학사과정에서 노인간호학 교과목이 대부분 2학점으로 운영되는 현황임을 고려하여[
본 연구에 참여한 대학의 학부 노인간호학 교과과정을 미국 NGNA의 핵심교과내용에 의해 분석한 결과 연구에 참여한 대학의 70.0% 이상에서 노인간호의 개요, 노인인구학적 특성, 노화이론, 노화과정, 노인대상자의 권리, 전문직 책임, 간호활동의 법적 해석, 정책개발, 노년기 건강문제에 대한 내용을 포함하고 있었다. 15년 전 노인간호학 교과과정에 대해 조사한 연구결과[
그러나, 미국 NGNA에서 제시하고 있는 노인간호학의 핵심교과내용과 비교시 우리나라의 노인간호학 교과내용에서 간과되고 있는 내용이 여전히 많았다. 노인간호의 법적/윤리적 이슈 영역, 노인간호의 행정적 이슈 영역, 노인간호연구 영역, 노인간호서비스 전달체계에 대한 교육내용이 여전히 부족한 상황이었다. 본 연구결과로 제안하는 표준교과과정에서는 노인간호 윤리와 법 영역과 노인간호 서비스전달체계 영역을 대분류에 포함하고, 노인간호 윤리와 법 영역에 노인간호의 윤리적 측면, 법적 측면, 노인학대를 포함하였다. 또한, 노인간호 서비스전달체계 영역에 노인간호 서비스의 연속성, 노인장기요양보험제도, 급성기 노인간호, 재가노인간호, 요양시설 노인간호를 포함하여 현재 노인간호학 교과과정에서 부족한 내용을 보완할 수 있도록 하였다. 반면, 노인간호 연구영역과 노인간호의 행정적 이슈 영역은 대분류에서 제외하였는데 그 이유는 대부분의 대학에서 노인간호 연구 영역은 간호연구 교과목에서, 노인간호의 행정적 이슈 영역은 간호행정 또는 간호관리 교과목에서 관련 내용을 포함하고 있으므로 이번 노인간호학 표준교과과정의 내용에는 포함하지 않았다.
또한, 본 연구를 통해 조사한 국내 노인간호학 교과과정 운영 현황 결과 노인의 건강문제 영역에서 당뇨병, 암, 후천성면역결핍증, 전립선질환은 별도로 다루는 대학이 없었으며 요실금은 단지 3개교에서만 다루는 것으로 나타났는데 이는 당뇨병은 내분비계에서, 암은 신체 각 체계 내에서 다루기 때문인 것으로 판단되고, 후천성면역결핍증은 우리나라에서 발생빈도가 상대적으로 낮기 때문인 것으로 사료된다. 전립선질환과 요실금은 노인 환자에게 매우 흔한 질환이므로 본 연구의 결과로 제시한 표준교과과정의 노인의 건강문제와 간호 영역에 포함하였다.

본 연구는 국내 노인간호학의 대표적인 학술단체인 노인간호학회에서 연구를 수행하였고, 노인간호학회 전문가 그룹이 도출한 핵심역량, 교과과정의 대분류, 중분류, 학습목표를 전국의 노인간호학 교수에게 의견수렴 과정을 거쳐 최종 핵심역량, 교과과정의 대분류, 중분류, 학습목표를 도출하였다. 본 연구를 통해 국내 학부 노인간호학 교과과정에서 달성해야 할 핵심역량과 노인간호학 교육의 방향제시가 될 수 있는 표준교과과정을 개발하였다는데 그 의의가 있다. 본 연구는 학부 노인간호학의 강의계획서를 통해 국내 노인간호학 교과과정 현황을 분석하였으므로 강의계획서에 제시하지 않은 소제목 등의 내용은 현황분석에 반영되지 못하였다는 제한점이 있다.
